The Practice works collaboratively with our local federation - Durham Dales Health Federation www.ddhf.co.uk - to provide a range of community services to our patients, such as:
- Community Wellness Team: a team of nurses and other healthcare professionals who help support people living at home and in care homes
- Community Psychiatric Nurses: who provide rapid access psychiatric support in the Practice for patients who need it
- Extended Primary Care Access: which allows patients to be seen on evenings, weekends and bank holidays in a local hub, by calling NHS111
- Pharmacists: who can help Practices manage workload relating to medication changes and reviews
