The Practice works collaboratively with our local federation - Durham Dales Health Federation  www.ddhf.co.uk - to provide a range of community services to our patients, such as:

  • Community Wellness Team: a team of nurses and other healthcare professionals who help support people living at home and in care homes
  • Community Psychiatric Nurses: who provide rapid access psychiatric support in the Practice for patients who need it
  • Extended Primary Care Access: which allows patients to be seen on evenings, weekends and bank holidays in a local hub, by calling NHS111
  • Pharmacists: who can help Practices manage workload relating to medication changes and reviews

Health Visitors

A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.

The Health Visitor holds walk-in clinics at the Practice every 1st and 3rd Thursday of the month between 9.30am and 11.30am.  An appointment is not necessary.
