Attached Primary Health Care Professionals - include the District nurse, Midwife, Health Visitor, Community Psychiatric nurse, Social Worker, Counsellor, Physiotherapist, Community Matron, Podiatrist and other Specialist nurses (PCT).
We offer an annual check-up to all patients aged 75 years and over—please book an appointment to see your GP or practice nurse.
You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website.
If you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of SSP (statutory sick pay).
It is up to your employer to decide whether you are incapable of work. A medical certificate, now called a 'Statement of Fitness for Work’ (see below) from your doctor is strong evidence that you are sick and would normally be accepted, unless there is evidence to prove otherwise.
You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist. Your employer will decide whether or not this evidence is acceptable. If your employer has any doubts, they may still ask for a medical certificate from your GP.
The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help you return to work sooner by providing more information about the effects of your illness or injury.

Vaccines that are NOT available on the NHS can be obtained from a private Travel Vaccination Clinic: Yellow Fever, Japanese encephalitis, Meningitis ACWY, Tick Born encephalitis, Hepatitis B, Rabies, Cholera and Antimalarials. As these are not available on the NHS, you must be prepared to pay a fee.
Please note we are not a Yellow Fever Vaccination Centre (YFVC) - you will have to find your nearest YFVC to obtain this vaccination. See www.nhs.uk
If you have left it late you may have to attend a private travel clinic for ALL your vaccinations as we only hold a limited supply of travel vaccines.

1. Please ensure you fill in the above form with all the information requested including anything else you think is important for your Practice Nurse to know.
2. Some vaccines or travel medications are not available on the NHS and so you will be charged cost of non-NHS vaccines/travel medication + VAT + P&P (that the practice may have been charged to obtain supplies).
3. Follow the instructions given in the TVS Leaflet above.
4. Depending on availability of vaccines/travel medications and/or type of vaccinations required, you may be asked to return for further appointments.
5. We are NOT a yellow fever vaccination center.
6. There is a £12.00 incl. VAT travel vaccination service fee. This will be in addition to any other costs as explained in (2) above.

The Practice is required under the terms of the latest GP Contract to allocate all our patients with a Named Accountable GP, who will have overall responsibility for the care and support our surgery provides.
You do not need to take any further action. If you wish to be told the name of your accountable GP, please ask our Receptionist when you next visit the surgery.
Please note: Having a named GP does not prevent you from seeing any other doctor at the Practice.
You are unlikely to see any notable change in the way care is delivered to you by our practice. **You can continue to see the GP of your choice.**
These responsibilities will be carried out within the normal opening hours of the surgery and do not change the way you or other patients currently access care outside of these hours.
You must continue to book appointments with the practice in the usual way. If you require an appointment with a GP urgently and are unable to book it with your ‘named GP’, please book in with another available doctor/duty doctor at that time.
Please note that despite our GPs working with other health and social care professionals to deliver your care needs, they do not take on the responsibility for the care others deliver to you. If you have concerns about any care provided by a health and social care professional outside of the practice, this should be raised directly with them, or their organisation.
